Procedures for Requesting Data Sets
The limited dataset from the first three years of the UAB Study of Aging will be
provided upon receipt of following:
- A completed signed Limited Data Use Agreement
form. Note that “Recipient” is the person requesting the data.
- A brief description of the project for which the data will be used.
- A copy of the Institutional Review Board (IRB) approval for the study for which
the data is requested.
Note that the data use agreement sets forth the following conditions:
- Data are to be used only for the described and approved research project
- The agreement is non-transferable
- No attempt will be made to identify individual study subjects
The UAB Study of Aging investigators have made a substantial commitment to establish
and maintain this data base. Collaborative relationships with outside investigators
are invited, and researchers are encouraged to contact the UAB Study of Aging investigators
to determine if their proposed research overlaps with work already underway.

South, Birmingham AL 35294-2041. On receipt and approval of the request, a CD-ROM
with the data set will be mailed to the requestor.
